Financial

Our practice accepts cash, check, and major credit cards. We will submit your insurance forms for you and accept most major insurance providers. Please call our office to confirm your specific insurance plan is accepted. Payment plans are also possible and can be discussed on request.

Cancellation

If you need to cancel or reschedule an appointment, please provide a minimum of 24-hours notice. We ask for this advance notice so that we can offer this appointment time to another patient. A cancellation fee may be charged in the event the patient does not show up for an appointment or provide notice.
